Help me understand my CAIPS

I would appreicate any help in understanding my CAIPS notes
PSDEC 1 SELDEC 1
BEDC 0 FINDEC 0
MEDICAL STATUS CODE: A
MEDICAL CODE: M03 H1 D1 T1 S1 E1
PAPER FILE SENT TO PA FILED BF'D 24-05-2011
Also in the notes it says
PLACED ON LIST 4/2011 Q 12
