I got my passport stamped and they have given me a Medical surveillance form for TB. Can any one explain me what this form is all about.

When you arrive in Canada, the IO after "landing" will give you the information that you have to report to the local TB clinic for surveillance.  See: http://www.cic.gc.ca/english/resources/manuals/op/op15-eng.pdf appendix B   and Section 6.4 and 10.
