u have to fill all the forms given on the site cic.gc.ca alongwith the fee and send it to NS, Sydney. All the details r there on the website under federal skilled workers. doctors come under NOC 3111 and 3112. u can check that on the NOC list. if any specific info u want u r most welcome. evryone on this forum will help u.
