Dr.
You need to send ORIGINAL documents, application forms and fee to NS Office in Canada. Before sending documents check whether they are complete, here is the checklist, send ONLY listed:
http://www.cic.gc.ca/english/pdf/kits/forms/IMM5612E.PDF
Follow this link to download complete application kit
http://www.cic.gc.ca/english/information/applications/skilled.asp
Read the guideline carefully before submitting your case

Hi,
if you are sending your application to CIO, Sydney, NS. You do not need to send any document with that. They only assess your "Initial Eligibility" from your application.
If you are eligible, then they would transfer your file to your local visa office and the visa office would require a complete set of documents + updated application from you.
At this stage, you only need to send your application forms + fee

AOR: April'09
Docs Sent: July'09 (Local Visa Office CHC - London)
Docs Rcd: July'09
AOR: 04th Sep'09
Since then e-Cas status "In Process" and Waiting for Meds Request
