finally I got reply from the embassy 

Dear Sir:
 
Please note that your spouse's file is pending the result of the background check. Once received we will be able to continue processing. In the mean while your spouse is required to undergo her medical exam again using one of our designated doctors  - list attached.

A background check is a standard procedure, it is neither good or bad that your spouse is being subjected to one, it's just part of the necessary processing.  They will do checks into the various addresses, work history and other matters of personal history that your spouse listed in their application.

Things that can cause this to take extended time periods include, but are not limited to: military service, membership in professional and political organisations and gaps in one's history.
