Download the form (B4 and B4a) from this website:

http://www.cbsa-asfc.gc.ca/publications/forms-formulaires/menu-eng.html

Usually people attach a computer generated list (by excel) with the B4 form, instead of using B4a.  B4a is just a continuation of B4.
