Hey everyone-  can someone shed some light on the above mentioned form.

My wife just got her passport back from the Caracas Visa office with the a copy of this form along with the Family Sponsorship application....

Does this mean she's been approved to come to Canada?  What are the next steps?

Is there a timeframe for her to leave her country?

Hi ,
That should be IMM5292B -the COPR (Confirmation of Permanent Residence ).
The time frame of travel is according to her convenience but within the expiry date of visa.
There should be a covering letter from Visa Office ( instruction letter  )  along with the passport and document, stating what are the next steps and the validity of her visa. If not, one year from the date of her medical is the visa expiry date, or her passport expiry date which ever is earlier.
