Hello Everybody,

Need some help in filling up the CAIPS/GCMS form. I will be requesting the said form as well as I'm the sponsor.
Section A: Does my husband need a consent letter inorder for my request to be complete?
               I ticked hard copy (paper), what is this "examine copy of information in a government office" for?   
Section C: Whose full address on the cheque or money order, my residential address printed on the cheque, the bank's address or their address?

1.  You need authorization from your spouse if you wish to get his information.
2.  Only if you want to look at the complete file do you check view.  Since it is an overseas case, you would only see the file that was sent to Mississauga.

More questions:
1. I would like them to send it via pdf but I'm not sure if our file is less than 25 pages. we've been through appeal & back plus ARC. Can I put a note that if more than 25 pages, let them send the hard copy then?
2. IMM5475: Is this the consent letter to be included in the GCMS/CAIPS Request or a formal consent letter is enough?
3. does the only way to send is by mail. Is there any fax number to send the filled forms?
