1.  For your own, NO, for someone else YES you need their authorization.

I think she was wondering if the consent form needs to be witnessed (i.e., does a third party need to witness that the proper person signed the consent form?).

you mean your signature on consent? my signature was not witnessed and everything went just fine. but that was a CAIPS application. I don't think there should be any difference with FOSS.
