Forms for Accompanying Partner/Spouse for Student Visa

I got accepted for a 2-year diploma course from a university in Canada for the January 2010 intake and I would like to have my common-law partner to accompany me for the duration of my studies.

I will appreciate it if you can clarify to me aside from the study permit application and the supplementary form together with the statutory declaration, what other forms should we accomplish so that my common-law partner will obtain the visa? Or is it enough for me indicate in the study permit application form that I will be accompanied by my partner?

Also, how much fees do I have to arrange for my application as well as my partner?
