Hello I am filling out the forms and i have a couple questions at the moment, probably more to come though.
What is a UCI number?
What is a National Identity Document? is it a social security number or a drivers license or what?

Hi Dusty,

UCI number =Unique Client Identifier number

If you have a National Identity Card issued by your government give its number. Otherwise leave it for NA
It is not passport or driver's license.
