Foreign Travel Form

Hi everyone, i got medical papers for immigration to canada under skilled worked category. They asked me to send passport and complete the foreign travel form, but didn't got foreign travel form in the package. Where can i get that form?

If you did not get it in the package you can download from the cic.gc.ca website.

I just made a travel form myself in MS Excel format in my PC  and submitted. It was accepted and I got PR accordingly.
 If you wish you can do it, if there is any problem to get the form.  You have to specify the dates of travel, (from - to). country visited, purpose of visit. ( ex: tourist/visit, study, job, pilgrimage, treatment etc). Then sign at the bottom as a declaration.
