hello guys... tumawag ako knina sa call center and i spend 7hundred sa tawag.hirap pla maka connect dun..nways sked na ko for pik up tom..but still i have a one question to make sna masagot nyo po agad..un pong about sa  check list gumawa po ba kyo ng sarili nyo na check list?2 kc ang pinapapasa ng agent sa call center gumawa po q isa ung check list ng mga sinubmit q tpos ung isa na check list ung nasa website ng canadian embassy..ok lng ba ung ganun? o dpat 2 copies na lng ng gnawa q na checklist..anu po ba ang tama...help po.. tnx..

ay meron pa pla q tanung san ilalagay ung checklist sa ibabaw ba ng envelope o isasama ko sa loob ng envelope na isasara?bukas na po ung pik up sna po me sumagot kaagad..tnx..po sensya na nagiging demanding po q..tnx anyway..
Hi Sis:
 Yung sakin gumawa ako ng sariling checklist ko(2 copies) hindi ko na isinama yung checklist ng canadian embassy . Then isama mo sa loob ng envelope sis bale yung sakin after the cover letter yung checklist ang next na nilagay ko followed by the other documents as listed in the checklist para organize at makita agad ng VO. hello sa lahat ^_^ newbie lang po ako dito... may mga questions sana ako regarding medical requirements na accredited ng canada:

1. Saan po yung best hospital or clinic na pwedeng magpamedical aside from St. Lukes?
recommended ng mga sis ang St. Lukes. ako sa Nationwide, ok dn daw sa Timbol.
2. Ano po yung mga requirements na dapat dalhin sa medical?
Yung MR tsaka 2 pcs id, pati photocopy ng passport ata? tgal ko n kasi ng medical. wait mo nlng MR mo.
3. How much po yung cost ng medical nila?

4,050 PHP

Additional Question po, kapag po ba naka receive ng AOR it means kailangan ko ng magpamedical?

Usually sabay yung AOR tsaka MR.

Pasensya na po sa dami ng tanong ko, first time lang po kasi Smiley

Aside from the official checklist na included sa documents, i made my own. actually parang table of contents lng naman hinihingi sayo nung call center agent. kumbaga according to order of appearance ng documents sa envelope ilalagay mo dun. ako kasi nag code pa ako para sakto talaga. like A-001 (birth certificate), A-002 (marriage certificate).. mga ganun.

Yung checklist/ table of contents ang unang-una dapat nilang makikita sa loob ng envelope. wag mong ilalagay sa labas ng envelope. diba may instruction sayo ano nakalagay sa labas ng envelope? yun lang dapat.

REQUIRED DOCUMENTS
For Regular, Immigrant, Working, Student, Visitor Categories
Passport/ national ID (SSS, postal ID, voter's ID, PRC ID. driver's license, school ID) and birth certificate
Two (2) PHOTOCOPIES of passport/national ID and birth certificate (see format)
6 pieces recent passport photos (white or blue background)
Medical Instruction from the Embassy (Embassy Form 1017)
Photocopy of Embassy Form 1017
For Sponsorship Immigrant Category
Passport
6 pieces recent passport photos
Appendix C or D Medical Instructions form
Whenever applicable, please bring the following:
Old chest x-ray films not less than 3 months prior to the medical examination
Detailed medical certificate regarding a previously-treated or currently managed clinically significant illness which should contain the following information:
duration and date of treatment
surgical procedure or medical intervention done
complete final diagnosis
outcome of treatment or treatment plans and prognosis, if available
 
OPENING HOURS
Reception Area      7:00 AM

Sputum Collection      6:30 AM

X-ray            7:00 AM
Laboratory Reception   7:00 AM

For the convenience of those who come early, the guards may allow applicants to enter St. Luke's Medical Center Extension Clinic premises as early as 4:30 AM but the schedule of operation of each working area will be observed.
 
MEDICAL EXAMINATION REQUIREMENTS
Age   Required         Examinations
0-4 years old      Physical Examination
5-10 years old      Physical Examination, urinalysis
11-14 years old   Physical Examination, urinalysis, chest x-ray (CXR)
15 y/o and above   Physical Examination, urinalysis, CXR, HIV, Syphilis
 
MEDICAL FEES
15 & above - Php 4,200.00
11 - 14 yrs old - Php 2,200.00
0 - 10 yrs old - Php 1,750.00
Senior Citizen - Php 3,360.00
 
REMINDERS:
Applicants are advised to take their meals prior to coming over for their medical examination since food is not allowed within the premises and all the initial blood and urine tests do not require a fasting state. There are restaurants near St. Luke's Medical Center Extension Clinic and drinking fountains are available within the building.
Females within the reproductive age group are advised not to schedule their medical examination during their menstrual period. Otherwise, they will be required to come back one week after the last day of their menstruation.
 
Note:
Medical results and documents will be released not on a first-come first-served basis but depending on the physical examination, x-ray and laboratory results, which will be conducted and processed in accordance with the guidelines set by the Canadian Embassy - Health Management Branch.
The medical report, and chest x-ray are directly submitted by St. Luke's Medical Center Extension Clinic to the Canadian Embassy in Makati. For offshore applications, medical reports and x-ray films are submitted via courier.
 
CANADIAN RECEPTION (5th floor)
Submission of Requirements
Filling-out the applicant's Patient Data Sheet (PDS)
Photo Encoding and Patient Data Encoding
Cashier (5th flr) - payment of fees
After mo po magbayad, punta kana sa 3rd flr for x-ray or sa 4th flr for urine/blood exam
then balik kana sa 5th flr sa Vital Signs Table
then sa US counter tatawagin din name mo for room assignments (Physical Exam)
Then TAPOS NA PO. Smiley Thank you! ^_^

sabi kapag spousal visa/open work permit hindi na daw kailangan dumaan ng PDOS, basta meron kang hawak na OEC, LMO, Work Permit  (ng hubby mo), check mo yung discussion about for pinoys spousal open work permit...

no PDOS unless dumaan ka ng agency. read the first few pages of this forum kasi dun ko ata pinost kung saang government agencies ako tumawag.

Hello, hello! I just have a question for those parents with school-aged children who were granted study permits.

Sa mga nagpresent ng Letter of Acceptance from school, naka-indicate po ba sa study permits na ibinigay ng Immigration at port of entry yung name ng school?

This is a concern for me... I was told by my employer that I'll be relocating to another city sometime after July. Kapag humingi ako ng Letter of Acceptance from one of the schools in my current city and Immigration officer indicated it on my son's permit, baka hindi sya makalipat ng school.

I used to hold a study permit when I first came here in Canada in 2009 nakalagay talaga sa study permit ko na I'm not supposed to attend any university apart from UofT. Clearly, ito yung iniiwasan kong mangyari for my son.

Sa mga hindi po nagpresent ng Letter of Acceptance, naka-"open" po ba yung institution and location sa study permit?

Any advice/suggestion? Thank you so much in advance!!!

hello po...bago lng ako s forum n eto at ang laki po ng tulong s mga kagaya ko n waiting p rn s visa....halos pareho tau ng timeline March 19 and 20 rn naforward sa CEM ang medical nmin ng mga kids ko....hoping na sana magkakasabay tau mka receive ng visa! Thanks po sa lhat ng mga walang sawa na mga advises!

hello joju are u new in the forum?anyways welcome....are you asking about cenomar?well base on my experience you can ask someone to get you a cenomar as long as they have your authorization letter a valid id of yours and of coz a valid id of the one getting a cenomar for you..
