This letter is a very old format that ND VO uses for medical requests. They have not bothered to change it with the times! These days the DMP takes a digital image of yours at the time of X-ray & imprints it with the X-ray shot. So negatives are not required.
Speak with your DMP when taking the appointment for medicals, he/she will clarify it. Take only the 5 passport size photographs.
I tend to agree with Yukon here.. seems logical...
anyways who gets negatives these days?
