Finally!!! Final step

After a year and half i got my passport requested through buffalo,
i have few questions that i wanted to double check before i send it,
1- i am living in the US legally, can i send the residency fee in USD instead of CAN dollars, i read somewhere that the fee is 415USD instead of 490 CAN, but i cant find the link?
2- for the pics requested, do i need to write sth on them like DOB Name File # or just send them with nothing?
3- anyone had experience sending the passports using USPS express mail with an inside encelope that is self-addressed for buffalo to send the passports back in it?
4- do i need to fill a form or sth and send it also, i didnt get anything beside an email from buffalo?
