Filling out application form to change condition(work permit)

Hi! I change employer as live-in caregiver and now i' m applying for a new work permit, my current work permit is still valid for another year. In the 1st page of the application form I marked letter "C"-an initial work permit or extension of work permit. In the 2nd page of the application form D-My Request, should I answer nos. 10 and 11? Should I mark "change conditions" and continue on answering no. 12? Or this only applies to those applying for another temporay resident permit? Please help! I really need to make sure of what to answer in this area so I could send my application. Thank you so much!
