Its a good thread. But is it filing to CIO to some specific CHC? If you clear it, we can share it properly.

Hello, I am submitting my application tomorrow to CIO.
I am little unclear about "2-self-addressed mailing labels". What does this mean?
I am sending my documents thru FedEx, so does that mean I should include another packet which address's me?
