@   betsy lahat to kailangan original ba Huh

Hi dawncmartin,

The best thing to do is get certified true copies of the medical reports you have from the institution where you did your tests. Major hospitals have Medical Records office from which you can request for copies of the laboratory tests and all other pertinent reports regarding a procedure that was done for you.

The medical certificate would have to be issued by your doctor and the reports would serve as attachments.

Hope this helps.

Hi, Rich!

We also have the same experience with CIIP.  We sent an email with our documents as registration for the seminar.  They said to us that November sched is full.  Still waiting for the next sched hopefully this December.  Kelan natapos medical mo? Were still waiting for further notice.  Natapos medical nmin ng wife ko last October 04 but submitted sa embassy on Oct 13, & 19 according from Nationwide.

Hi Norbie!

Nice to hear from you. We almost have the same timelines in our application. I've originally applied November 2009 pero binalik sa akin kasi mali yung VISA office na specify ko dun sa unang form. Buti na lang minor mistake lang. So we corrected it and sent it again around January 2010. We are still processing our medicals kasi nga they are requesting for medical certificates from our doctors (Cardio for me and Endoc for my wife). My Cardio wants to do a threadmill test pa before he issues the certificate so medyo bumagal process namin. Tsaka meron akong new born na hinabol sa application. Di pa nadating yung medical forms nung bagong baby so we have to wait pa din for that to finalize the medicals. Palagay ko mauuna ka sa amin.

Let's keep each other updated to share info na din as we proceed.

Regards to all

Rich

Hello sa lahat,

I am a newbie here and I am glad that Filipinos are helping each other through this forum. May tanong po ako - meron ba sa inyo ang nag-apply after June 26 (the day Canada changed some of its immigration rules)? If, so how was it? I passed all my documents to the intake office at Nova Scotia on 10/19/2010. No communication yet from my lawyer as to any acknowledgment of receipt.

the CIO received my documents/application on Oct. 18, 2010 (under the June 26, 2010 rules).  According to BPI Bank, my bank draft has already been encashed by CIO. what does it mean? BPI has yet to provide me with the exact date of encashment. Later daw I will receive a call from BPI re the date, busy pa daw sila. Kaka-anxious naman bigla. Mukhang mabilis sila mag-encash, pero sabi sa ibang forum, it will take 33 weeks daw bago ma-receive ang AOR from the date of encashment.

psychnars,

In my case I used credit card. I submitted my application to CIO May 13,2010 and the payment was charged to my cc by Sept 7,2010. But I received the notice of approval from CIO by Oct 5,2010.So it took a month from charging the payment to issuance of notice.

To EVERYONE:

Does anybody have first degree relative in Canada? What document(s) did you submit to Canada Embassy as proof of relationship? Because in my case, my uncle in Canada (brother of my mother) has no Birth Certificate. The NSO instead issued Affidavit of No Records. Is this enough to prove our relationship? Please I need your help. Thanks

thanks pisces18k for your reply, sa case nyo po kasi before June 2010 kaya iba ang processing time ng CIO. sa case po naming mga "on or after june 2010 policy", medyo unpredictable pa. kahit sa ibang forum, wala pa rin nakakareceive ng AOR kahit na-encash na ang BD 2-3 months ago.

Your uncle have to file again sa municipality where cia pinanganak. bring the affidavit of no records. magpapagawa ulit siya ng late register birth certificate then he will advise na i forward ang late register b.c sa nso. thats the time na pwede na siya kumuha ng nso copy but it will take long - 2 months processing bago cia makakuha ulit ng copy sa nso.

Hello pisces18k,

We have the same situation. My first degree relative is my uncle. Problem is he was born in 1939 and he doesnt have any record of birth in our municipal hall. Thus the civil registry made a certificate saying that the record of birth was destroyed by floods, war and termites. I figured it would be futile to register my uncle's BC because he is 71 years old and had been living in Canada for 40 years. So I went to the church where my mother and my uncles was baptized . I was able to obtain a certificate of baptism with registry number and date of baptism. Also included is my grand parent's name. Not sure if this is alright but Certificate of baptism is seemed  viable to prove relationship.
