Fees Paid on Internet - Do you fill out receipt or just submit print-out as is?

I paid our fees online.  I printed the receipt.
There are spaced for Name, client date of birth (who is this? the sponsor?), address, postal code etc.

Do you fill this receipt in before including in the application?  Or just submit as is?

You have to fill it out.

Most will say you should normally put the sponsor's info in there. Others will say put the info of the person who paid the fees (whoever's credit card it was, for example). I filled mine in with my info (I paid, I'm the applicant) and had no issues.

where do i get that information?

You won't have one yet, just leave it blank.  That's what my husband did, and there was no problem.
