I think that the Demand Draft / Bank Draft has the name of the "Beneficiary" and the "Name of the Bank in Canada", and the issuing Bank's Name and details.
The Demand Draft is already Paid for, so writing the name of the person in the Fee form might not be that important as it is for Credit Card. IF you are also the account holder (one of), I guess it would be ok to write your name. I dont think that there is name of the Payee on the DD. At least wasnt there for me.
The DD is like a cheque issued by Bank (not you), that holds value, as it has already been paid for, or something like that. The Bank thingies are always confusing to me
