Hello to all of you,
me and my boyfriend applied for NOC 4151 and they received our application in February, 27th, 2012. and they tried to make payment in March,8th, 2012. but because of invalid expiry date of credit card, they returned to us.
Now, we gonna resend it and we need answer. If we need to refill only main application form with the new date or we need to refill all application forms with this new date or it enough to send just the first one because we have changes there because of the UCI number.
Please, help us.

We had almost the same situation too. Here is what we did: We filled out a new form (with the UCI number) and attached it to the original one. Also, for our NOC they have had changed the number and as we were not sure which one we should use, we attached a new application with the new NOC as well. So in short, we had the originally submitted forms along with with the new ones. We figured that they should be able to decide which one exactly they would need.
A piece of advise- if you are paying with a credit card, make sure you contact your bank / credit card company and inform them that you would expect a charge from a different country. I believe they can put your status as "traveling" to ensure that they will not block the payment. You may need to call your bank every other day to ensure they are not blocking the payment.
