I need some help!!!!

1. Where can I get the FBI fingerprint form besides requesting to the FBI in WV? I've requested one about 3 weeks ago, but haven't received the form yet, and the time is passing by.

2. Once the fingerprint is done and ready to request the personal background check, where should I have the result sent to? to my mailing address? or to the Canadian Consulate where has been conducting the process?

I would really appreciate if anyone could assist me on these matter.

1.  You can download the form from the FBI web site and then print it on heavy card stock.
2.  Have it sent to your home address.
