AFAIK, in Canada they dont recognize Middle names. but they are requiring us to write down our name as it appears in our passport...mine was simple because i only have a single name followed by my middle name, then surname, hence i wrote them down in each of the boxes, the second box i wrote my middle name....as i have said just follow the way your name is written in your passport... say in first box given names: John Carlo, middle name: Santos, surname 3rd box: Cruz

thanks! finally my application forms are almost done.

In the Generic Application form, some blank boxes were grayed out once you click Yes or No for an answer. How about those blank boxes that is not applicable to you to have an answer but writing NA or N/A is not possible (keyboard input) like the following:

1. UCI - I'm a new applicant so I don't have this
2. Citizenship(s) item #2 - I don't have dual citizenship
3. P.O. Box - Don't have this, leave it blank or type "None" (keyboard input possible)
4. Apt./Unit - Don't have this, leave it blank or type "None" (keyboard input possible)
5. District - Don't have this, leave it blank or type "None" (keyboard input possible)
6. Alternate Telephone No. under Ext. - Don't have this
7. Fax No. - Don't have this

Do I have to put answers like NA or None (in handwriting) after I have printed it or leave it blank?

thanks future Canadians!

Does anybody knows if is possible to request a new copy of the AOR? I received it by email but never by post, in those days the strike of Canada Post was in place, and I think that probably it got lost.

I need the original letter for the OHIP renewal... They won't accept any emails.

PER stands for what?

Hello Hinnajamal

I copied this from someone here in the Forum. It explains you.

You will either get PER (postivie eligibility review) or NER(negetive eligibility review)
CIO will issue PER if they find out your score is equal or above 67
CIO will issue NER if they find out your score is below 67(NER means rejection of your application)
Usually it takes approximately 4 months to get either PER or NER
