I will get married in 2 weeks in Philippines and my future wife will use my family name after our wedding (for ex, when she will proceed her passport). I'm just confused about what name we should put when we will complete the immigration forms, should we put her family name or my family name?

If your wife is changing her passport BEFORE you submit your application, her name on the forms must be her married name, as the name on the forms must match the name on the passport.

If she changes her passport AFTER you submit the application, she will need to advise the visa office of her new passport information.
