Pls attach COP, Your Returns, Place of Practice Proof, Experience as Practising CA, Reference letters from your clients (though its a Prof Misconduct), Types of Consultancy services you are offering, Service TAx Assesment returns., your appontment as Auditor in Cos. Banks Etc.
