I have a question...I was married before (12 years ago) and the PR application sponsor info asks for her address and birthdate.
As I haven't spoken to or seen her in over a decade I lack this information. With no idea where she is or anything about her what am i to
do in filling out the forms? I have the wedding date and diveorce dates but that is all.
Any info will be helpful!

If you do not have the information write a letter to describe why.
If you have the wedding and divorce documentation it should be fine.

Have you tried to facebook her? Google her? Maybe that can give you some leads. If not, what you can say in your letter is that you have not been in contact with her for 12 years, you have used social networking sites to find her but could not (proving that you did try to look for the information) and you do not know any of her relatives/friends/etc.

haha funny I've been divorced for 9 years, I had to email my ex (he's a professional so contacted him through is official website) Asking for his Bday and address (for government forms) All I got back was
"I have nothing to say to you, and I never will"
And he dumped me! !
I just wrote a letter ... I tried his parents in England too, never got a response...
