If you're work permit is still valid then no need to fill out the application to change conditions etc. just the application for temporary resident visa form on the website and the other documents required. Are you going home? If so, be adviced that they have encountered lots of delay for visa processing in the consulates, in their website it states you must atleast send the documents 3 months before your travel date. Hope this helps!  Smiley

If you are in Canada with a temporary work permit and need to apply for a TEMPORARY RESIDENT VISA to travel abroad, please provide:

A photocopy of your current permit and
A letter from your employer or other proof of current employment.

You need to send a copy of your work permit, letter from your employer that you're still working for them, and follow the instructions of the consulate where you`re sending your application to on how to get back your passport. No need to send copy of the ticket. Courier is safe. Which visa office are you sending your application to?
