I guess I didn't realize that you have to actually "qualify" to become a citizen.  What is required?  And is this standard for all countries?

Everyone from immigration process has to perform and pass a Test in order to become a Citizen; it is the same as US

And yes it applies for all the countries.

That is right.  However, I believe that it is much harder to qualify for a US visa/migration compared to applying at Canada, New Zealand or Australia.  If we pass the required no. of points required in these countries, then we have interview and medical exams aside from fulfilling the documents needed, we have lots of chances to go but applying in the US is pretty much difficult unless you're a good nurse or teacher or has relatives petitioning for you.
