Education - diploma transcript question

I have a diploma in Catering Management from South Africa (1994) which actually lists all the modules that were completed as well as the time taken (3 years). Would this be considered a combination diploma/transcript?

I know that the requirements are for something in a university sealed envelope, but I am getting a blank wall with the hotel school as they have since merged with another university and seem quite unable to provide something from 15 years ago.

provide 3 things...

1)notarized copies of whatever you have
2)explanantion letter fo lack of transcripts
3)any communication - e-mail or physical that states that they cannot provide because the college changed university affiliation, or whatever the case may be...
