Dear All Member,
I have come to know some respected members that I need to take letter from Licensing Authority of home country if I want to apply driving license in Canada? Can any one let me know what exactly I have to write from licensing authority of home country?

My second question is what exactly child immunization record consist of and from whom I have to get it? As right now I don't have any immunization card of my children which is most important document required to shown in Canadian School to get my children admitted with them?

Kindly suggest?

Thanks for sharing such a helpful information? I have already download the form but having some confusion about the sentenced mentioned "This Form - Immunization Record Form - will be issued by the immigration/visa officer handling the immigration application" in first para.

I am confused when I have already got the P.R. how can approach the Canadian Visa Officer to get issued it from him?

Can I get it completed and certified from Quality Health Professional / Family Medical Doctor and directly submitted to the school in which get admitted the dependent children?

What should be level of education of Quality Health Professional / Family Medical Doctor? Should he be BAMS, MBBS or M.D? having their own practice or being appointed in Government Approved Hospital?


Is this filled form is enough to present before school authority or do need any other immunization documents?

Getting the Child Immunization record should not be a issue at all

Call up some one in your home back in India and let your family doctor prepare such record or you can
contact your child's doctor

I am sure you might have taken care of all immunization for your kid while in India, its just you are
missing that piece of paper...
