doubts about case specific enquiry

As u all know..ND is holding my pp from last four months..so i am thinking to go for cse
there are some spaces in the form...on which i have some doubts..plz suggest me the right way to fill it out


1)Applicant's Information   {dis section is about me (getting sponsorship) i think...as i am applicants for pr application.. rite??

2)*Family Name:              ( what should i fill here... as in past i never used my surname ..and the acknowledgment latter i received from             
                                       ND didn't mention my surname. i open my ecas with my full name)
 
3)*Given Name:            dis must be my full name  Aman**** kaur
   
*Date of Birth:
       
*Nationality:

Client ID # or File #:         
   
Telephone number:

4)Enquirer Identification                {here also i should fill my own info??}
   

*Relationship to Applicant:     
*Family Name:
   
*Given Name:
   
*Email Address:

5) Consultant ICCRC No:   ( wats dis)

2. i am nt quite sure..
3.Put in ur full name as thats wat u used while applying..
4.yes, ur own info
5. If u applied thru consultant, ask him for it.. if nt, leave it blank..

I have replied to the best of my abilities.. but i wld advise u to seek other's opinion too..
