my question is "what i need to fill in application as sir name?"
can i keep it blank?

u should write what ever is there on passport to maintain same data

As i mentioned in the PM, write exactly as appears in ur passport...Mandie

BUT how can she leave the Family Name column empty?  OR write N/A (not applicable) in column of Family name???   I havent before come across such thing. BUT i guess the advice is good to be inline with the Passport.

I thought it was a bit weird too not to have surname/family name on passport...but in order to create consistency, he needs to fill his details as appears on his passport....Mandie

Also DONT LEAVE the FAMILY NAME column blank, write NOT APPLICABLE or N/A.

  i was late to read ur replies

my family name is "alookkaran"
my father name is "lazar"
my name" shijo "
together  it is "alookkaran lazar shijo"
 
passport office has  print it in the place of "Given name as Alookkaran Lazar Shijo"

and kept family name blank.

i filled my application family name as "Alookkaran" and given name as "Alookkaran Lazar Shijo" and sent today.
