Documents required to include wife in immigration file

Hi All,

Can someone please provide me some information for adding wife in immigration file? (applied under AINP-H1B Visa Holder Category)

I got married in December 2010 and received my medical in May 2011.
1.   What type of documents should I send to include wife in immigration file?
2.   Is P O Box Address acceptable for immigration file? (Planning to change home address and provide P O Box Address for immigration file)
