How did you submit " 2 self-addressed mailing labels" in no. 8 of "Document Checklist IMM 5612"?

I mean should I print my address on a sheet of paper and submit it with my application?

Printed my address and just cut it to a suitable size that can fit an envelop, you can also get label template from microsoft. otherwise, just print and cut....hope this helps
