Documant Checklist 5491 vs Appendix A Immigrant Checklist

The Appendix A Document Checklist - Immigrant  asks for the Sponsorship Questionnaire IMM 5540. What's up with that? This document the 5540 is part of the Sponsorship Application package. Should I include this document twice? One with my Sponsorship application and and other in my spouses application for
permanent residence?
The Appendix A Checklist states that "If there is a conflict between this guide and any other versions or publications, this document and it's
instructions take precedent and are to be followed."

So to be on the safe side should I go ahead and include a copy of my 5540?
