hi coolbreeze,

what is the Transcript certificate from your medical college. and what is national ID?

It is a statement issued on the letter head of your medical  college signed by the DEAN.
It states your name , dates of your enrolment and graduation , the name of the degree, dates you did internship.

It also mentions the transcripts subjectwise. Meaning total hrs of studying for each subject.
eg. anatomy 900 hrs, Medicine 900 hrs, ENT 100hrs.
Your academic registrar or your dean's office will know about this.

Skip National ID , just write not applicable.

hi coolbreeze, In the checklist, it is given as "Transcripts: original transcripts for all successfully completed studies, if available." I have only one set with me issued by college. Should we send originals to them?

No. Just send a copy. I had one original too, so I sent them a copy which was accepted. Have it notarized like other copies.

hi coolbreeze, in Schedule A/Background declaration 8.Personal history, last column says Name of employer, Company, School, Facility, as applicable. what should I write in that last column for the period that I was unemployed and for the period I travelled in other countries. And next one in the same application(9), in EG 7 it says to write " I have never been a member of any organization or Association"  rather than writing not applicable and in application it is suggested to write 'NONE'  which one I have to write there?

hi coolbreeze or somebody can help me with these doubts. Some of them may be silly but once I post the application, I can't do anything about it. I am planning to put "at home" or "at friend's residence" for the last column in the above mentioned personal history. And I wrote "NONE" for membership of any organization or association. Does our medical registration comes under membership? Please bear with me for few days till i submit my application. Please give some suggestions.I might be asking few more questions before I submit my application.

For organizations NONE is just fine. I wrote NONE too.

For the "unemployed" period, just write unemployed if at home or travelling if abroad. Be truthful. What matters is the period that you are showing
as "employed" is well-documented and gets you the required points under "work experience".

Hello,
First ,you must visit the canada immigration website with a cool mind and read Federal skilled worker immigration thoroughly.
Download the application package on your PC and read everything.
Everything is very nicely explained.
The website is

http://www.cic.gc.ca/english/pdf/kits/forms/IMM5612E.pdf

The following documents are mainly required;

1. Passport copy
2. Municipal Birth certificate copy
3. Marriage certificate if married.
4. Original IELTS scores
5.. Original Police Clearance from Passport office or local district office.  It should not be older than 3 months when you send it.( you may send this one later to visa office when asked for)
6.. Copies of your educational certificates, starting from class X  to the Dental degree.
7.. Copy of Transcript certificate from your medical college.
8.. Medical registration copy.
9.. Proof of Funds ( bank deposits, savings statements )
10. Copy of experience certificate(s), reference letters.
Very important.;- Your experience certificate must include the duties the canadian immigration
requires as a dentist. If it does not , your application will be rejected. To get an idea of the duties, read;
Dentists diagnose, treat, prevent and control disorders of the teeth and mouth. They work in private practice or may be employed in hospitals, clinics, public health facilities or universities.

(Description from Human Resources and Skills Development Canada's National Occupation Classification, used by Canadian immigration officers, to assess an applicant's work experience.)

Examine patients' teeth, gums and surrounding tissue to diagnose disease, injury and decay and plan appropriate treatment
Restore, extract and replace diseased and decayed teeth
Perform oral surgery, periodontal surgery and other treatments
Clean teeth and instruct patients on oral hygiene
Design bridgework, fit dentures and provide appliances to correct abnormal positioning of the teeth and jaws, or write fabrication instructions or prescriptions for use by denturists and dental technicians
Supervise dental hygienists, dental assistants and other staff.
Dentists may specialize in such areas as oral and maxillofacial surgery, orthodontics, pediatric dentistry, periodontics, endodontics, prosthodontics, oral pathology, oral radiology or public health dentistry.
11. 6 photos.

You must include working hours in the letter ; for full-time work , it has to be at least 40 hrs per week.  Dates of joining and leaving. Salary drawn.

Oh c'mon!!! When you are unemployed, there can't be any company. Just  write NONE.
