It seems that you might be able to get your medical without a passport but really you will still need one at some point. Talk to the US embassy or consider returning to the US to get one. There is no way to have legal status in Canada without a passport.

Why are you still at a loss?? You will need both the medical report and passport for your PR application. So just decide which one you want to do first. Either apply for your passport  now following the link given above and then get your medical done. OR get your medical done now and apply for your passport afterwards. If you do the medical before you get your passport, bring your driver's license and the print out from the web which states the instructions for the medical (ie. if no passport is available, bring an official identity document).

I thank every one of you who responded. Which leads to my next question--How do I get a passport while in Canada with an expired driver's license and no record of landing here?

why not to contact US embassy about this questions - they have all the rights to help you with obtaining your passport an maybe driving license too.
