Hi all!
Minor question... which address does one use for the Appendix D medical form? Should it match the principal applicant's mailing address as per the application? Or does it not matter since the IMM 1017 EFC form is given to them right there, therefore, nothing else should be mailed?
Any advice?

Married Sept '06 after 4 yr LDR. Applied via outland ap in March 2007 - refused Jan '08 due to inadmissible adult "dependent child". Appeal filed Jan '08; still waiting for resolution.

crap, I think I put my wife's parents address down on the Medical form. I put my address down for her mailing address on all the other paper work. Do you think this could present a problem?

Is your wife staying with you? Are you applying outland or inland?
Also, as a side note, according to the CIC website, Appendix D (Medical Form) is now Appendix C, since they took out the Police Check for some reason, which was the original Appendix C.

No she is not with me right now. She's from Thailand and she's renting an apartment there for the time being. On all the forms I put her apartment down as her residential address and my address as her mailing address. Except on the medical form and maybe police form I think I might have put her parent's address down. We applied for a temporary resident visa before the PRV and the embassy has said they will issue her the TRV when my sponsorship gets aproved (hopefully in the next few weeks). We applied outland and she will be comming to stay with me on a TRV until the PRV gets approved.
