My wife is principal applicant for me for my permanent resident visa for Canada. she got visa after that i submitted my file. CIC proceed my file and they ask me for medical examination but problem is that they put my wife surname instead of mine. so it would create any problem in future?
should i contact CIC for this issue? and my wife didn't change her surname yet. and she got visa on her old name Passport only..

there is no problem if ur wife continue wid her parental surname........ my case is bit similar to urs.... i m the primary applicant and i haven't change my surname after marriage.... cic don't have any problem with that....

than u should contact cic as early as possible... they have written in the letter attached to medical form that if u see any discrepancy in the medical form regarding ur name or address than u should inform them as soon as possible..... they will issue u new form....every thing is mentioned in the letter.........read that letter carefully.......

i have some case like that , all what to do is to send an email to them and attach your passport copy

Hi, Inform CHC at the earliest for the change. Else you will face a problem at the time of landing. IO cross verifies the details on your passport with CORP.
