According to CIC you do

How or where do I obtain one?
Applicants should apply for a certificate at:

Deputy Commissioner (Security)
Commissioner Afghan Refugees
Banglow No. 155
Phase V
Hayatabad
Peshawar

What is your question?  If they just sent you something to notify you of your status but they did not ask you to do anything or send anything then you don't have to do anything.  When they ask you to do something or send something, you do it.

You can see processing time for spousal at http://www.cic.gc.ca/english/information/times/international/05-fc-spouses.asp#asia

If the application was just send to Islamabad, you are looking at 6 to 9 more months unless you are very lucky and are faster or unlucky and slower.
