Very truthful..and I would like to add something with regards to working as a dental assistant, one has to be HARP (Healing Arts Radiation Protection Commission) certified, meaning that you have to be licensed to practice radiography as a dental assistant.
You can get that certificate by submitting your radiography curriculum from your dental school in addition to your dental school transcript and degree certificate to their address:

5700 Yonge Street, 3rd Floor
North York, Ontario
M2M 4K5
Canada
Thanks hondubai that's good to know!

Birth certificate is must however, i heard NDVO may accept 10th certificate instead of BC
following docs are needed to prove relation between your Uncle and your mother:
1. Notarized Birth certificates (must) of both your uncle & you mother showing same parents name
2.photocopy of his  Record of Landing (IMM 1000), Confirmation of Permanent Residence or Permanent Resident Card.
3. IF he is a citizen then proof of Canadian citizenship, such as a photocopy of pages of a Canadian passport or Canadian citizenship card
4.Additional docs to prove that your uncle is currently living in canada:
you can submit either his income tax/electric bill/bank statement etc.

regards,
Dr.Hasib

Thanks Dr. Hasib,
we have applied for my uncle's BC,may get by 15th July.

Hello all

Just received an email from cic. My application was rejected for dental therapist as i did not show enuff evidence that i performed all the duties and caps for dentist was already filled when my application reached cic on 19 May. Guess i have to reapply again. Is there any chance that they will send back my application and all the documents and if not, when i reapply, can i just ask them to refer to the previous app for all the documents? Since i am self employed as a dentist, what further evidence can i give to them about the duties i performed, other than providing them invoices, income taxes statement, audit report etc. Lastly, i want o add my partner in my app but in my previous app ,i mention that i was single. Would this pose as a problem, as i was hoping not to include her until i moved there since time was limited when i send in all my app in may and could not gather all her documents that time. Pls advise. Thanks

Thanks, i hope so too. Well i placed 2 noc on my app in may 2011, coz i knew that chances that caps for noc 3113 will reached 1000 when my app reached cic is pretty high. Not knowing then whether dentist will still be in the list in july, i took the chance and placed dental therapist as one of the noc figuring that heck, m pretty sure that it doesn't take a genius to know that a dentist is more qualified than a dental therapist ( sori no offence to dental therapist out there), but i guess i was wrong, it does take a genius to figure that out Grin. Was so happy when c.c got credited on 26 may, but today i received the sad news:( Oh well back to reapplying again. Fingers crossed this time.

Hi,
1.CIC most probably will not return your application package. but they will certainly refund your payment
2.You can't ask them to refer to the previous app for all the documents. You must arrange a new set of docs+application forms+ payment.
3.If you have experience as a self-employed person, provide copies of:

A)Submit a Statutory Declaration. This is a written statement declared to be true in the presence of an authorised witness(notary public). This should be verifiable and be supported by other evidence of your employment(reference from other Dentists)
The Statutory Declaration — must include the following details:
a)the full name and address of the company and any telephone, fax numbers, email and website addresses
 b) the name and position of your referees (other dentists) and   contact number of  them
 c) details of the exact period of employment
including:
• whether permanent or temporary
• whether full-time or part-time
• position(s) held -
• duties undertaken(must match with the duties mentioned in NOC website), and the salary earned

B)  Reference certificates from other Dentists (atleast 2 )
C)business/company registration documents
D)proof of business conducted with clients (invoices, bills, client references)
E)income tax returns, and/or
F)proof of your licence to practice

4) you can add your partner...this would not be a problem
