Hi everyone.

I'm a bit confused about how we should fill out the declaration of common law union.

1. the part that says I _______ and my partner ______ of (city)____country____ etc

Hmm, that part should be simple but at the moment we have no single place of residence. We are traveling together through SE Asia for 3 months and when we are done I am departing to Canada while she is staying here in The Philippines.
How would you fill it out?

2 Does it have to be notarized through a commissioner of oaths?

Our 1 year of living together is Oct 2nd. I wanted to put we've been cohabiting together Oct 2nd/2010 to Oct 20th/2011 but if we get it notarized I guess we will have to do it after Oct 2nd but before my flight to Canada Oct 20th

3. It asks what documentary proof we have. My answers is all the other evidence we're subitting such as text messages, pictures, airline tickets etc....was that the correct way to answer or is it only talking about legal type evidence?

I think you should put the address where the two of you lived for the longest period of time - the hotel where you stayed from last October till you started travelling.
You should get it notarized.
