Cover Letter- to NewDelhi for adding childs passport

Any one with formats of cover letters for sending childs passport copies to High commision Delhi,
please let me know here thanks all.

It should be a simple letter, I wrote to Buffalo:

Dear VO:

I want to add my new born baby to my permanent resident application under category SW-1. Following are the details of the dependent I want to add to my application:

As required I have also attached following documents & updated forms with this communication or mail:

1) Processing fee for child : $150*1 = 150
2) Child's birth certificate (if you need english translation write down notarized copy of original and  English Translation)
3) Copy of child's valid passport
4) 3 recent photographs of child
5) updated IMM0008 Generic for my self and updated IMM5406 “Additional Family Information” form for myself and my spouse. 
6) Updated proof of fund
if you are under QSW then you also need a “Certificat de Sélection du Québec” for your child ( in his or her name).
