confusion on Supplementary Identification Form (IMM 5455)

You must order the Supplementary Identification Form (IMM 5455) and the HPM Receipt Form (IMM 5401).
Must Order!! Can't I just download this!

No, you cant download the supplementary form. It takes about 2 weeks to get the form.
The reciept form is only if you want to pay through a bank. If you pay online you dont need it.

Kind of strange though. A supplementary form can't be offered online? totally beats me.

Good news: You can now download the form the website of Citizenship and Immigration Canada. See explanations at Operational Bulletin 383 - February 1, 2012.

you can download IMM5455 BUT YOU have to order IMM5401
