Hi there...

i need help to fill out the form imm 5406 for the PR card....

"Please submit an Additional Information Form for your mother and your father. Submit one form each."

Juz got this answer from the Embassy...im confused.....do they have to fill out the same form as I do...or one for my father and one for my mother.....is that the same form they have to fill out....or are there any other forms for parents of applicant???

Im the only one who is applying....my parents arent applying...then why do they need a form for them too?Huh

Each person who is applying and is 18 years of age or older must fill out one of these forms.  You do not need to fill out one of them for your father and one for your mother because like you said, they are not immigrating.

But they are asking for their information too....were do i have to write it down on the form...thts the confusion actually??

This is the form: http://www.cic.gc.ca/english/pdf/kits/forms/IMM5406E.PDF

The first line is for you.  The next is for your spouse if you have one, the next 2 lines are for your parents.  That was section A.

Section B is for your children if you have any.

Section C for your brothers or sisters if you have any.

Unclear.

So if I'm the applicant, my name should go in the first line of  Section A, then my mother's and my father's names in the third and fourth lines respectively.

What about the second line? My husband is also my sponsor, do I need to enter his name here? or is  this line reserved for a situation where the person being sponsored is NOT the sponsor's spouse? A bit confusing.
