CONFUSED AFTER RECEIVING CAIPS NOTES!! HELP PLEASE!!!!

Dear Friends!!
After checking the CAIPS Notes I am confused a little bit. Its came to my knowledge, that London Visa Office has not updated my Application. All the data in CAIPS is as per my initial application, let me explain for your convenience:-
Initial application was sent in June 2009, At that time many documents were missing. My kids were studying in lower grades, PCC and IELTS were not attached with the application. My wife's passport was expired and my consultant mentioned in application "Under renewal"
In Initial Application My Consultant mentioned that my dependents are not accompanying me to Canada.
In September 2009 V.O asked for full documents and updated application. Updated Application with all documents were sent to V.O in December 2009. In updated application I changed my kids grade to next class and we sent the renewed passport copy of my wife and mentioned all required details on the application as well.
My consultant mentioned in Updated Application that my dependents will Accompany me to Canada!!
IN CAIPS VISA OFFICE ACKNOWLEDGE THE RECEIPT OF UPDATED APPLICATION IN DECEMBER 2009
Now after checking the CAIPS Notes I found that all the information in CAIPS Notes is based on the initial application. School Grades for my kids are old, passport No. for my wife is blank, further its mentioned in notes that my family is not traveling with me.
Only they added few things from updated application like, PCC, IELTS and they kept all other information as per Initial application.
NOW MY PROBLEM IS:-
1- I am very near to PPR, will they ask for my passport only or they will ask for the whole family?
(Medical for whole family is finished and all the due fees for whole family has already been paid)
2- Shall I ask them that my family is traveling with me?
3- Shall I ask them to update my data as per the updated application?
4- If I ask VO to update the information, what are the chances of further delay in PPR??
5- What should I do in this situation??
I am sorry for this long Story!
Regards
