Confused about Criminal record procedure

It says you can download a coverletter and fingerprint card from the FBI website. What is the coverletter? I think I got the fingerprint card...it's an application with the spaces for the thumb, index, etc. I don't see the coverletter. So after my husband gets this done, I guess we send all the info. we get from the police plus the prints to the address for FBI and wait to get something back. Do we have to pay when he gets the prints at the police station? we're in the US. I know we have to pay a fee to send to the FBI.
