I don't know if this simple question has already been taken up.

In the Philippines, our passport has the first name, middle name (mother's maiden name) and last name. Example: Richard Tan Guillermo

Question: Do we need to include Tan as a given name? Or just Richard Guillermo? I believe that Canadian Passport does not include the middle name.

Please help.

Use the name that is on your birth certificate and wedding certificate. I would include your middle name Tan in it.

If you are referring to what name to use when filling in the forms, it is best to go with the name on your passport - it creates less confusion in the long run.

My Canadian passport has my middle name.
Fill in the name as it is in your passport. You can attach a note stating it is actually your mother's maiden name if you want, although I think the visa officers in Manila should be aware of this Philippine naming custom.

On IMM0008 Schedule 1, Questions 1,2 & 3.
#1 Will be the official name as per documents, passport, birth certif. etc.
#2, you name written in your native lenguaje (with carcteres or any symbols that your lenguaje have)
#3 Other namethat you use if you sign or write your name without the Tan, or other nicknames etc,

You can choose to whether or not to have you middle name on your Canadian passport.

You can also choose to have your passport issued in your maiden, married, or even your common-law partner's surname.
