We are going to apply for our US born child's Citizenship Certificate but we seem to have an identification issue.  On the "Checklist" it clearly states that the following forms of ID are requires:

•A birth certificate which lists your parents (issued by responsible government authorities in the country where you were born)
•Proof that one or both of your natural parents were Canadian citizens when you were born
•Two (2) pieces of personal identification, such as a driver's licence and a health insurance card
•Other documents, such as a marriage certificate/divorce certificate and/or a legal name change certificate (if applicable)

The first two are simple and the last really is not applicable...but how about that third item?  We have an insurance card...but a driver's licence?  What other forms would be suitable?  Almost tempted to get her a Costco Card!

We would be thankful for any input at all!

The ID is for the canadian parent. So you could use your health card, or drivers licence. I used my passport and health card and it was fine through the Canadian Embassy.  Its to establish your identity as a Canadian.

I also have the same issue, I haven't used my BC Health Card (Which by the way does not have a picture ID on it) for years now, don't know if it's valid or expired, but do I send in copies (Both sides of the card i presume) with my application??

And I've never had a Canadian Drivers licence do I send in my current and only non-canadian drivers's licence photocopy in instead?

Would my Canadian citizenship card which has a picture id be sent in instead of the driver's license?
