citizenship application questions

Hello I will be eligible to apply for citizenship in a few months time.  I have the following questions to which I need guidance.

Question - Request for different name to appear on certificate - is not applicable to me so should I leave it blank or write N/A in the space provided? Should I also do this for all questions that are not applicable to me such as 5B - Mailing address if different from home address? Should i leave it blank or write N/A to all questions not applicable to me?
